Forms

Build multi-page forms that ask each person only what applies to them, look up addresses and vehicles, and take uploads and payments. Your own admins make every change, and each one is approved before residents see it.

Pages, questions and branching

All of it is set up in the form builder. An answer can skip a page, show a question, or end the form on a page that sends the resident somewhere more useful.

  • Text, dates, choices, addresses, streets, and uploads
  • Answers checked as an email, phone number, postcode, or registration
  • Branching between pages, and ending pages that stop or submit
  • Guidance that uses earlier answers, and a check-your-answers page
  • Errors summarised at the top of the page and shown beside each question
Form builder
The Casebridge form builder open on a “Make a complaint” form. Its first page asks which service the complaint is about, and branches from there; a Council Tax page stops the form.

Look-ups, uploads and payments

A postcode becomes a confirmed address, and a registration becomes the vehicle’s make, colour, and tax status. Uploads are checked as they arrive, and a fee can be taken on the page you choose.

  • Address search and map pins from Ordnance Survey
  • Streets limited to your area, with their USRN
  • Vehicle details from the DVLA
  • Uploads checked for type and size, and that each file is what it says it is
  • Payment through GOV.UK Pay, confirmed before the form submits
Public form
Greenbridge Borough Council’s “Make a complaint” form on its “What happened?” page. The resident has entered a date in day, month, and year boxes, and written that their green bin has been missed three weeks running.

Publishing

Drafts

Editing a form changes a draft. Residents keep using the live version until the new one is approved.

Approval

A draft is submitted for review, then approved or sent back. Approval is refused if the draft changed after it was submitted.

Preview

Go through a draft as a resident would. No Case is created and no message is sent.

Version history

Each published version is kept as it went live, with who changed it and who approved it.

Branding

Forms carry your organisation’s public name, logo, and colours.

On your own website

A form has its own address, and can also be embedded in a page on your website.
